This section will guide you through the major content additions from the Stir of Dawn, Strife and Ruin, and Wounds of Eventide updates, detailing how to access them, what new items and abilities you can acquire, and strategies for overcoming their unique trials.
Stir of Dawn (New Game+ and The Penitent One's Path)
This update introduced the much-anticipated New Game+ mode, known as "The Penitent One's Path," alongside significant new content accessible in both standard and NG+ playthroughs.
Accessing Stir of Dawn Content:
- New Game+ (The Penitent One's Path): After completing the game once, select "New Game+" from the main menu. This mode features increased difficulty, new enemy placements, and access to the Three Penitence Challenges.
- Miriam's Challenges: These challenges are available in any playthrough (new game or NG+) once you reach the Graveyard of the Peaks. Look for a new portal near the top-right exit to Jondo. Interacting with the portal will transport you to a series of combat and platforming trials.

- Three Penitence Challenges: Exclusive to New Game+, these are selectable at the start of your NG+ run. Each Penitence drastically alters gameplay:
- Penitence of the Bleeding Heart: Replaces your health bar with a system of "Bleeding Hearts." Taking damage removes a heart, and they do not regenerate automatically. Flasks of Bile restore a fixed number of hearts.
- Penitence of the Unwavering Faith: Removes Flasks of Bile. Instead, your Fervour regenerates much faster, encouraging heavy use of Prayers and Bile Flasks now restore Fervour.
- Penitence of the True Guilt: Upon death, you lose a significant portion of your Tears of Atonement and Fervour, which remain at your death spot. Dying again before recovering them results in permanent loss.
- Miriam's Challenges Rewards: Successfully completing all of Miriam's platforming and combat trials will reward you with the Miriam's Anklet Rosary Bead, which significantly increases your damage output.
- New Boss: Sierpes: This formidable foe is encountered in the Echoes of Salt area, accessible after completing a specific questline involving a new NPC. Defeating Sierpes grants you the Heart of the Holy Purge, a powerful Mea Culpa Heart.
- Voice Acting: The entire game received full English voice acting, enhancing the narrative experience.
Strife and Ruin (The Bloodstained Crossover)
This update brought a crossover event with Koji Igarashi's Bloodstained: Ritual of the Night, introducing a new area, boss, and a familiar face.
Accessing Strife and Ruin Content:
- Echoes of Salt: To begin this questline, you must first find the new NPC, Miriam (from Bloodstained), in the Library of the Negated Words. She will appear after you have defeated at least three Mea Culpa Heart bosses. She will task you with finding five specific items scattered across Cvstodia.

- New Area: Echoes of Salt: This ethereal, blood-red dimension is accessed through a portal opened by Miriam. It features unique enemies and environmental hazards.

- Miriam's Dagger: A new Mea Culpa Heart obtained after defeating Bloodstained Miriam, which allows you to throw a spectral dagger.
- Reliquary of the Bleeding Heart: A powerful Relic found within the Echoes of Salt that provides a passive health regeneration bonus.
- Skins: Completing the questline unlocks a new skin for The Penitent One, inspired by Miriam's design.
Wounds of Eventide (True Ending and Final Boss)
The climactic final free DLC, Wounds of Eventide, introduced the true ending to Blasphemous, a new area, and the ultimate confrontation with the true antagonist.
Accessing Wounds of Eventide Content:
- True Ending Path: To unlock the Wounds of Eventide content and the true ending, you must first complete the following prerequisites:
- Defeat Crisanta of the Wrapped Agony in the Archcathedral Rooftops.
- Obtain the Holy Wound of Abnegation from the Library of the Negated Words (after defeating the boss there).
- Obtain the Holy Wound of Compunction from the Mother of Mothers (after defeating the boss there).
- Obtain the Holy Wound of Contrition from the Sleeping Canvases (after defeating the boss there).
- Return to Crisanta in the Archcathedral Rooftops. She will now be in a different state, and interacting with her will initiate a new sequence.
Key Features and Rewards:
- New Area: The Mound of the True Penitent: A hidden, desolate area that serves as the final approach to the true ending. It features new, challenging enemy types and environmental puzzles.
- New Boss: Eviterno, Father of the Penitents: This incredibly difficult boss is the true final challenge of Blasphemous. He boasts multiple phases and a wide array of devastating attacks. Mastering his patterns is crucial for victory.
- True Ending: Successfully defeating Eviterno and fulfilling all other prerequisites will unlock the game's true ending, providing a more complete narrative conclusion to The Penitent One's journey.
