The Brain of Mensis is a grotesque and optional boss found deep within the Nightmare of Mensis. This encounter is unique as the Brain itself is initially invulnerable, protected by a barrier. Hunters must manipulate the environment to expose and damage it, making this a puzzle-like boss fight that tests observation and execution.

To initiate the fight with the Brain of Mensis, you must first reach the Nightmare of Mensis. After defeating Micolash, Host of the Nightmare, you will find a lever. Pulling this lever causes the Brain of Mensis to drop from above into an arena below. The Brain is initially protected by a shimmering barrier. To break this barrier, you must attack the Brain directly. However, it will periodically unleash a powerful arcane attack that can inflict Frenzy. The key is to dodge these attacks and strike quickly when the barrier is down. Defeating the Brain of Mensis yields the Living String and the Tier 3 Moon Caryll Rune.
