The Living String is obtained by defeating the Brain of Mensis in the Nightmare of Mensis. After defeating Micolash, Host of the Nightmare, and Mergo's Wet Nurse, you can pull a lever to drop the Brain of Mensis into an arena. You must then defeat this grotesque entity. Upon its defeat, you will receive the Living String. This item, along with consuming at least three One Third of Umbilical Cord items, is necessary to access the true final boss and achieve the secret ending of Bloodborne.
