Acquiring this rune is a key step for many builds, especially those focused on Arcane or Bloodtinge damage.

To obtain the Tier 3 Moon Caryll Rune, you must progress through the Nightmare of Mensis. After defeating Micolash, Host of the Nightmare, and Mergo's Wet Nurse, you will have the opportunity to interact with the Brain of Mensis. This involves pulling a lever to drop the Brain and then defeating it. Upon its demise, you will be rewarded with the Living String, and crucially, the Tier 3 Moon Caryll Rune.
