Congratulations, intrepid explorer! If you've reached this section, you've likely seen the credits roll on your first playthrough of Cave Story. But the adventure doesn't end there! Cave Story is renowned for its hidden depths, alternative endings, and challenging post-game content that will test your skills and knowledge of the game to their limits. Prepare for a true test of mettle!

The Best Ending Path: Escape from the Island

The "true" or "best" ending of Cave Story requires a specific sequence of actions throughout the game, culminating in a challenging escape from the collapsing island. This path unlocks the Blood Stained Sanctuary, the ultimate post-game challenge.

Prerequisites for the Best Ending

• Save Curly Brace: This is the most crucial step. After defeating Balrog in the Sand Zone, find Curly Brace in the Waterway. You MUST drain the water and give her the Air Tank (found in the Mimiga Village reservoir after defeating Balrog) to allow her to breathe. Later, in the Core, after she collapses, you must carry her to the Plantation and place her in the bed in the Mimiga Village doctor's house (the one with the red roof).
• Retrieve the Tow Rope: After saving Curly and placing her in the bed, speak to the Mimiga doctor. He will give you the Tow Rope.
• Find the Booster 2.0: This essential item is located in the Outer Wall. After acquiring the Booster 0.8 from the Labyrinth, you must navigate the Outer Wall to find the hidden room containing the Booster 2.0. This significantly improves your mobility and is vital for the later challenges.
• Do NOT Take the Gum Key: When offered the Gum Key by the Doctor in the Plantation, refuse it. This key leads to the bad ending.
• Acquire the Machine Gun: While not strictly required for the best ending, the Machine Gun (found in the Sand Zone, traded for the Jellyfish Juice from the Mimiga Village reservoir) is highly recommended for its versatility and power in the upcoming challenges.

The Last Cave (Hidden)

If you've met all the prerequisites, after defeating the Undead Core and being given the option to escape, you will instead be able to access the Last Cave (Hidden). This is where the true final challenge begins.

1. Accessing the Last Cave: After the Undead Core battle, instead of taking the normal escape route, look for a hidden path in the ceiling of the Core area. Use your Booster 2.0 to reach it.
2. Navigating the Last Cave: This area is a gauntlet of powerful enemies and environmental hazards. Your best weapons (Machine Gun, Spur, Nemesis) should be maxed out.
3. Boss: Heavy Press: This formidable boss guards the exit. It has multiple phases and requires precise dodging and consistent damage output. Focus on its weak points when exposed.
4. Rescue Curly Brace (Again): After defeating Heavy Press, you'll find Curly Brace. She's lost her memory but is otherwise fine.

The Blood Stained Sanctuary

The ultimate test of skill in Cave Story, the Blood Stained Sanctuary, is only accessible after achieving the best ending and saving Curly Brace. This area is a brutal boss rush and platforming challenge, culminating in a battle against the true final boss.

Accessing the Sanctuary

After completing the Last Cave (Hidden) and witnessing the best ending, load your cleared save file. Return to the Jail area. In the room where you first met the Mimiga prisoner, there will now be a hidden passage in the floor. This leads directly to the Blood Stained Sanctuary.

Sanctuary Challenges

The Sanctuary is a single, continuous gauntlet. There are no save points within, and if you die, you restart from the beginning. You will face:

• Boss Rush: A series of rematches against some of the game's toughest bosses, often in more challenging configurations. Be prepared for:
  • Balrog (various forms)
  • Grasping Hand
  • Ma Pignon
  • Ironhead
  • And more!
• Platforming Gauntlet: Interspersed between boss fights are extremely difficult platforming sections filled with spikes, falling blocks, and relentless enemies. The Booster 2.0 is absolutely essential here.
• Resource Management: Health and weapon energy are scarce. Use your Life Pots strategically and try to conserve weapon energy by using lower-level weapons on weaker foes.

True Final Boss: The Undead Core (Omega)

At the very end of the Sanctuary, you will face a significantly powered-up version of the Undead Core. This battle is a true test of endurance and pattern recognition. Its attacks are faster, more numerous, and deal massive damage. Focus on dodging and exploiting the brief windows of vulnerability.

Defeating the Undead Core (Omega) will grant you the ultimate satisfaction and a unique ending sequence.

Other Post-Game Discoveries

• Weapon Leveling: Continue to level up all your weapons to their maximum Level 3. Some weapons become incredibly powerful at this stage, like the Spur (fully charged) or the Machine Gun (for sustained damage).
• Item Collection: Ensure you've found all hidden items, such as the Life Pot upgrades (increasing your maximum HP), the Arms Barrier (reduces damage taken), and the Turbocharge (speeds up weapon charging).
• Challenge Runs: For the truly masochistic, consider challenge runs like:
  • No-Damage Run: Complete the game without taking any hits.
  • Weapon-Specific Run: Beat the game using only a single weapon (e.g., only the Polar Star).
  • Low-HP Run: Avoid collecting any Life Pot upgrades.
