Location: Operating Theater of the Old Clinic (End of Chapter 2)
Description: A spectral, elongated figure of a doctor, constantly whispering unsettling phrases. It moves quickly and can teleport short distances, making it elusive.

Phases & Attacks:
- Phase 1 (75-100% Health): The Doctor uses a 'Spectral Scalpel' attack, a quick, short-range lunge. It also employs 'Whispering Aura', a localized area-of-effect attack that drains your sanity if you stay within its radius.
- Phase 2 (25-75% Health): The Doctor begins to summon 'Spectral Nurses' (weak, one-hit enemies) to distract you. It also gains a 'Teleporting Lunge' attack, quickly closing distances.
- Phase 3 (0-25% Health): The Doctor's whispers intensify, causing faster sanity drain. It will frequently teleport and lunge, often chaining attacks. It also gains a 'Screaming Pulse' attack, a wider area-of-effect sanity drain that also deals minor physical damage.
Weaknesses:
- Light: The Whispering Doctor is vulnerable to bright light. If you have the 'Flashlight' (found in the clinic), keeping its beam on the Doctor will temporarily stun it and increase damage taken.
- Explosives: 'Pipe Bombs' (craftable) are effective for dealing burst damage, especially when it's stunned.
Strategy:
In Phase 1, keep the Flashlight on the Doctor as much as possible. Dodge the Scalpel attack and move out of the Whispering Aura. Use your Handgun for consistent damage. In Phase 2, prioritize taking out the Spectral Nurses quickly to avoid being overwhelmed. Use the Shotgun if they get too close. Continue to use the Flashlight to stun the Doctor before unleashing your attacks. For Phase 3, maintain extreme mobility. The Screaming Pulse can be devastating. Use any remaining Pipe Bombs when it's stunned. If you have the 'Mind-Calming Incense', use it to mitigate sanity drain. Focus on burst damage during its stunned phases.
Rewards:
- Doctor's Journal: Provides crucial lore about the ritual.
- Sanity Vial: Restores a significant portion of sanity.
- Story Progression.
