Upon reading the book, a cultist named Cook will appear and challenge you. He will explain that Boethiah desires a champion and that you must prove your worth by sacrificing someone to the Prince. To do this, you will need to lure a follower to the altar. You can either bring a follower with you or find one in the vicinity.
Once you have a follower at the altar, you must use the enchanted sword found on the altar to kill them. This act of sacrifice is what Boethiah demands. After the sacrifice, Boethiah will speak to you, revealing that you must now defeat her current champion, who is also present at the altar, along with several other cultists who have been summoned.
This will initiate a challenging combat encounter. You will need to fight your way through Boethiah's summoned warriors and her current champion. The fight can be difficult, especially if you are not well-prepared. Utilize your strongest attacks, spells, and shouts to overcome them.
Upon defeating Boethiah's champion and her summoned minions, Boethiah will be pleased with your performance. She will then bestow upon you a powerful reward, reflecting her favor.
