Survival in Post Trauma often involves careful navigation and puzzle-solving rather than direct combat. Players must utilize maps to avoid getting lost in mazes, make critical decisions during events like the subway train sequence to determine the game's ending, and strategically interact with the environment during boss encounters.

The game presents various scenarios that require different approaches to survival:

  • Maze Navigation: In the Underground Maze, Roman must use a map on the wall to locate two keys in the southwest and northwest portions of the maze. The map clearly shows walls in white and corridors as open spaces, with the entrance portal in the southeast and the key-unlocked door in the west-middle.
  • Subway Train Sequence: This segment directly impacts the game's ending.
    • To achieve Ending A, Roman must speed up the train, resulting in Jill's death. Controls involve circling the handle left (up on the directional pad).
    • To achieve Ending B, Roman must slow down the train, saving Jill. Controls involve holding the handle right (down on the directional pad).
  • Abomination Boss Fight: This is a puzzle-oriented encounter rather than a traditional boss fight. Players control both Roman and Freya.
    • Roman's role involves holding a lever to move a border for Freya.
    • Freya's role involves interacting with consoles on a high walkway.
  • Environmental Awareness: In areas like the School Courtyard, players must contend with challenging camera angles that can obscure paths. Roman needs to carefully observe his surroundings to find the correct way forward, such as running towards the camera and then turning to reveal new paths.
