The climax of Post Trauma presents players with a pivotal decision that directly influences the game's ending. This choice occurs within the "Subway Train" area, where you gain control of the train's speed, impacting the fate of Jill.
As you approach the section where Roman is inside a subway train, you'll notice that you can now influence its movement. The controls for the train are as follows: circling the handle left and right using the directional pad. Pressing up on the directional pad will circle the handle to the left, causing the train to speed up. Conversely, pressing down on the directional pad will circle the handle to the right, slowing the train down.
Jill will eventually appear on the front of the train, visible through the glass. Your interaction with the train's speed at this critical juncture determines which of the two primary endings you will achieve.
