Enemy Encounter Locations
Navigating the twisted environments of Post Trauma requires more than just a keen eye for puzzles; it demands an intimate knowledge of where dangers lurk. This section details specific enemy encounter zones, offering strategies and item recommendations to ensure your survival.
The Abandoned Apartment Complex
Your journey often begins or returns to the dilapidated apartment complex. Be wary, as it's a hotbed for early-game threats.
- First Floor Corridor (Near Apartment 103):
- Enemy: Grotesque Lurker (1-2 instances)
- Strategy: These slow-moving, shambling creatures are best dealt with from a distance. As you exit Apartment 103 after retrieving the Rusty Key, one will often spawn directly ahead. use the Pipe Wrench found in the kitchen of Apartment 101. Aim for headshots to conserve durability. If you have the Makeshift Pistol (found in the laundry room), a single shot to the head will usually dispatch them.
- Loot: Often drop Small Health Pack or Scrap Metal.
- Stairwell Landing (Between 2nd and 3rd Floor):
- Enemy: Screeching Harpy (1 instance)
- Strategy: This flying abomination will ambush you as you ascend. Its erratic flight pattern makes it difficult to hit with melee. Equip the Makeshift Pistol and aim for its body during its dive attacks. Alternatively, if you're low on ammo, hide behind the overturned furniture on the landing and wait for it to land briefly before striking with the Pipe Wrench. Be prepared for a quick follow-up attack.

The Submerged Sewers
Once you gain access to the sewers via the maintenance hatch in the apartment complex's basement, prepare for darker, more claustrophobic encounters.
- Main Drainage Tunnel (After Valve Puzzle):
- Enemy: Aquatic Abomination (1 instance)
- Strategy: This large, tentacled creature patrols the central drainage tunnel after you've manipulated the water levels using the Rusty Valve Handle. It's highly resistant to melee attacks. Your best bet is to use the Flare Gun (found in the sewer worker's locker, accessible with the Sewer Keycard). One well-placed flare will stun it, allowing you to run past. If you must fight, aim for the glowing sacs on its tentacles with the Makeshift Pistol, but be aware of its powerful slam attack.
- Loot: Drops Sewer Grate Key upon defeat, essential for progression.
- Collapsed Pipe Junction:
- Enemy: Swarm of Vermin (Multiple small instances)
- Strategy: These fast-moving, rat-like creatures appear in groups. They deal minor damage but can quickly overwhelm you. The Molotov Cocktail (craftable with Alcohol and Cloth Scraps) is incredibly effective here, clearing out entire groups. If you lack explosives, a wide swing with the Pipe Wrench can hit multiple targets, but be prepared to dodge their lunges.

The Derelict Hospital Wing
Accessing the hospital wing through the sewer's hidden passage marks a significant increase in enemy difficulty and psychological horror.
- Patient Room Corridor (Ground Floor):
- Enemy: Phantasmal Nurse (1-2 instances)
- Strategy: These ethereal entities are immune to conventional weapons. They will phase in and out of existence, making them hard to track. To defeat them, you must first acquire the Blessed Talisman from the chapel. Equip the talisman, then use its active ability when a Phantasmal Nurse is visible to briefly solidify it, allowing you to strike with any weapon. The Surgical Scalpel (found in the operating theater) is surprisingly effective during this window.
- Loot: No direct loot, but defeating them often reveals hidden pathways or items.
- Morgue Entrance:
- Enemy: Reanimated Corpse (1 instance, often accompanied by a Grotesque Lurker)
- Strategy: This encounter is a test of resource management. The Reanimated Corpse is slow but incredibly resilient. Focus your strongest attacks on it, ideally using the Shotgun (found in the security office). Aim for the head. The accompanying Grotesque Lurker should be dealt with quickly using the Makeshift Pistol or a swift Pipe Wrench combo to prevent being flanked. Consider laying a Trip Mine (craftable with Scrap Metal and Wires) near the entrance to thin out the numbers before engaging.
- Loot: The Reanimated Corpse drops the Morgue Freezer Key.
