Basically, enemies have three main states of awareness about Shay, and you can tell what's going on by looking at the color of the little icon on the far left of your health bar. Let's break 'em down so you can stay one step ahead.

Incognito (Blue Icon)

This is your default, chill state. When the icon is blue, guards pretty much ignore you. You can walk right past them, pickpocket, or do your usual sneaky business. They'll only start paying attention if they stumble upon something they shouldn't, like a dead body (oops!) or if they catch you red-handed doing something illegal. Basically, if you're not actively doing something suspicious, you're safe.

Investigation (Yellow Icon)

Uh oh, someone's getting suspicious! When that icon turns yellow, it means guards have noticed something a bit off and are now actively looking for you. They'll get a yellow marker above their heads and will start moving towards your last known position. If they get too close and spot you, they'll likely turn aggressive and you'll enter open conflict. Your best bet here is to break line of sight as quickly as possible. Duck behind some cover, use a haystack, or just blend into a crowd. If you can hide effectively, they'll usually lose interest and go back to their patrols.

Conflict (Red Icon)

Red means danger! This is open combat. When the icon is red, guards have seen you and are hostile. They'll attack you on sight. Your options here are pretty simple: either fight your way out and take them all down, or make a run for it. If you decide to escape, you'll need to break line of sight, find a good hiding spot, and stay hidden until the heat dies down and the icon turns back to yellow or even blue.
