To get FUSION #12, you'll want to head back to the Minish Village. Specifically, make your way to the house on the left side of the village, where you'll find Melari, the blacksmith. He's the one who can help you with these Kinstone fusions.

When you speak with Melari, he'll tell you he's looking for a specific Kinstone shard. If you have the correct shard in your inventory (it's usually one you've found from a treasure chest or a rolled-up bush), you'll be able to fuse with him. This is the key to creating FUSION #12.

After the fusion is complete, Melari will mention something about a treasure chest appearing somewhere in Hyrule. Keep an eye out for this! It often appears in the Hyrule Castle Garden, usually near the fountain area. Inside, you'll find a valuable reward that will help you on your journey.
