Complete Doom II Map 16 Suburbs walkthrough, including finding the Blue Skull Keycard and all four secrets.

Walkthrough
  1. 1
Immediately turn around to face and kill the Imps. Collect the Chaingun to your left.
  2. 2
Deal with the Revenants to the north; rockets are effective, but it's best to do so from outside after clearing immediate threats.
  3. 3
To the east, eliminate the lone Revenant. To the west, use your Plasma Gun or BFG to quickly defeat the Arch-Vile.
  4. 4
Explore the southern building. Enter the large door on the south side to find it filled with Imps; a BFG shot is recommended. Collect the health inside if needed.
  5. 5
Go through the smaller door. Kill the Cacodemon ahead.
  6. 6
Activate the red torch to raise the ceiling. Lower the counter below it, stand on it, and when it raises, shoot the eyes on the wall to unlock [SECRET 1/4], which contains ammo and a Backpack.
  7. 7
Lower the table with shotgun shells. Ride it back up and fire your pistol at the red flame object to unlock [SECRET 2/4] and obtain a Megasphere. Grab it before leaving the map.
  8. 8
Head south through the small opening and snipe the Imps on the wooden structure.
  9. 9
To the north of the Imps, use rockets to kill the group of three Revenants, leading them to the opening to avoid their rockets.
  10. 10
Kill the slow-moving Mancubus that follows the Revenants. Explore their cubby for ammo and a Partial Invisibility Orb.
  11. 11
In the cubby to the north, defeat the Arch-Vile and three Cacodemons using rockets, BFG, or Plasma Gun. Collect the ammo and Invulnerability Orb inside. Save this orb for later.
  12. 12
Head north and use the walls to take pot-shots with your Super Shotgun (SSG) at the two Mancubi on smaller structures. The first structure has a Supercharge Item, and the second has a box of shells.
  13. 13
Return to the toxic waste pit, ensure Imps are dead, and take the stairs up to find a Radiation Suit, multiple boxes of rockets, and the Blue Skull Keycard.
  14. 14
As soon as you grab the Blue Skull Keycard, a large number of enemies (Cacodemons, Imps, Mancubi) will teleport in. Run past the two cubby areas to the north and let them engage in infighting. Lead any pursuing Imps to the north and kill them with your shotgun if they don't fight other demons.
  15. 15
After about 35-45 seconds, grab the Invulnerability Orb you saved. Wade into the infighting enemies and use your Super Shotgun or BFG shots to clear them out.
  16. 16
Go to the Red Brick building on the eastern side of the map. Head to the east side and open the door. Step inside to trigger [SECRET 3/4]. Kill the three Cacodemons within.
  17. 17
Lower the northern wall in the Red Brick building to find and quickly kill a Revenant. Ride the wall upwards.
  18. 18
To the south, kill another Revenant from a distance near the blood pool. Head over to the ledge, drop in, and hit a skull switch to open another secret room, counting as [SECRET 4/4].
  19. 19
Inside this final secret room, collect the BFG 9000 and its associated ammo.
