Walkthrough
- 1Jump down and head right to a lighted area, then squeeze through a hole.
- 2Enter a large, dark, open area patrolled by Walker. Head to the middle and find stairs leading out of the water.
- 3Go up the stairs to a ladder, jump the gap, and climb up.
- 4In the corridor, head to the first path on the left. When the psycho charges, head left, then forward, right back to the main path to outrun him.
- 5Enter the next (third) path on the left and follow it left to escape the psycho.
- 6Squeeze through a narrow space to end the chapter.
- 7In the new area, look down the tunnel and into the blocked door on the left for the [_FEAST FOR FLIES_] note.
- 8Head up the stairs to receive the new objective: Reach the ground floor of the male ward.
- 9In the room, push the crate blocking a vent out of the way and crawl through.
- 10Exit the room and head left. Enter the door in the dark, then look at the left wall for writing to record the [_ORGANIZED TORTURE_] note.
- 11Continue down the hallway. Ignore the stairs on the left as they are a dead-end.
- 12Enter the room on the right with a body. A crate blocks a door; open the door and head left.
- 13Find and crawl through a vent.

- 16Find the gurney below an open ceiling tile and use it to climb up to the vents.
- 17Drop down into a room with a restrained patient as two psychos begin to break down the double doors.
- 18Quickly find the other door in the corner, push the crate out of the way, and take the other door as another psycho slams into a door.
- 19Push the crate across the door you just came in, then push the crate blocking the exit.
- 20Exit to the hallway and take a left. Hop the obstacle and enter the bloody room to the left.
- 21Push the crate by the door, then head to the vent.
- 22Drop down into a wide hallway. Run left, enter the room on the right, jump the desk, head left through another room, and keep going straight.
- 23Jump the gap to get away.
- 24Continue left (the door ahead is locked), then right. Head right, around the corner, up and over the desk and wall.
- 25Continue down the path, jump over the obstacle, and enter a slightly open door on the left.
- 26Hop into the dumbwaiter and ride up.
- 27After the scenes, break free and retrieve your camcorder.
- 28Exit the room and head left to a wide open room.
- 29Listen to the man (record!) and hide under a nearby bed when he starts going crazy.
- 30Keep recording the conversation between the doctor and the man to get the [_DR. RICK TRAGER_] note.
- 31Exit the main door of this room, head left, then head right.
- 32Jump to the vent in the ceiling (this is the elevator, which requires a key).
- 33Drop down and move the box blocking the door to the left, which opens the way to the previous area.
Tips
- Walker is dangerous but has trouble finding you in the dark if you stay far enough away.
- If you get seen by Dr. Trager, there are many beds to hide under. Running and hiding is your primary escape route.
- Dead-end hallways with darkened corners can also be used for hiding if beds are not an option.
- When escaping Dr. Trager after getting the note, be quick to exit the room.
