This portal is guarded by Rakinishu, and you'll need to have completed the quest The Search for Cain to make it appear. Don't worry, once it's open, it'll stay that way for any new games you create in that difficulty. Keep an eye on your minimap – that little yellow cross icon will pop up when you're getting close to the portal, making it way easier to spot.

Once you've found that portal to Tristram, step through it. Inside Tristram, make your way all the way to the western-most corner. You'll find Wirt's body there. Go ahead and loot him – you'll get a bunch of gold, and more importantly, you'll find Wirt's Leg. This is a crucial item for opening the way to the Secret Cow Level.

Now, head back to town. You'll need to grab a Tome of Town Portal from any vendor. It doesn't matter how many scrolls are inside it; any Tome will do.

Next, grab your trusty Horadric Cube. Place both Wirt's Leg and the Tome of Town Portal inside the Cube. Make sure these are the ONLY two items in the Cube.

With those two items in the Cube, hit the transmute button. Boom! You'll create a portal right where you're standing, leading to the Secret Cow Level. Pro Tip: Try to do this in a convenient spot. Opening the portal right on top of an NPC or your stash can be a bit of a pain to navigate later.

Tips

Once you step through the portal, you'll be facing hordes of Hell Bovines, all led by The Cow King. The main draw here is the insane enemy density – it's not always the best for finding unique items, but it's fantastic for runes, gems, charms, and socketed gear.
Keep an eye out for the Cow King's Leathers set. It only drops in the Secret Cow Level and, while not super powerful, it's a neat collectible for completionists.
