Step 2: Easy Difficulty Speedrun + No Item Box Uses + No Medkits + Ending 1
Complete the game in under 4 hours for the "Just Get Me Outta Here" trophy. Combine this with "Resource Manager" (no item box uses) and "Walk it Off" (no medkits, though 3 are allowed). Giving Mia the cure after defeating mutant Jack Baker results in Ending 1, which skips a boss fight and saves time. The "Circular Saw" unlocked from this step is invaluable for Madhouse difficulty.

Step 3: Madhouse Difficulty + Antique Coins
On Madhouse difficulty, enemies are more aggressive and spawn in different locations. Manual saves are required using Cassette Tapes. Collect all 33 Antique Coins, as missing one necessitates another Madhouse playthrough. These coins unlock powerful weapons and upgrades from cages, making the "Circular Saw" particularly useful for this challenging difficulty.
