    In the Operation Theatre Balcony, examine the PATIENT'S CHART on the shelf.

    Run to the far end of the balcony and step down the stairs to the right.

    Move along the right side of the walkway, turn off your flashlight, and choose to "Drop Down" at the end where there is no railing.

    In the Operation Theatre, ensure your flashlight is off. Listen to the radio static for nearby enemies.

    Turn around to face a Nurse near the operating table. There are three Nurses total on the east side of the table. If fighting, use a charged strong attack. You can lure them by standing still with the light off.

    Be careful not to bump into objects, as this will alert the Nurses.

    Examine the double doors to the Operating Room; they are chained shut.

    Turn east, cut the skin wall with your combat knife, and squeeze through the opening.

    In the East Staircase area, pick up the CHILD'S DRAWING (2/18) from the wall opposite the skin wall.

    Move up the stairs and examine the body on the gurney. Take the ++OPERATING THEATRE KEY++ from its hand.

    In the Operation Theatre, approach the chained doors to the Operating Room. Press LB when the icon appears, select the Operating Room Key, and use it to unlock the chain.

    A Swarm will emerge from an incubator after you take the toy; be prepared to fight them.

    Pick up the CHILD'S DRAWING (3/18) from the wall to the right of the mirror.

    Return to the Hallway Outside Operating Room via the east end of the Nurse Center, saving your game along the way.

    Approach Josh at the gate, open your inventory, select the Robbie Toy, and give it to him. A cutscene will play.

    Walk through the now open gate and pick up the CHILD'S DRAWING (4/18).

    Most doors down the hall are locked. Enter Room 206, which is open. The door will shut as you approach it.

    Take the CHILD'S DRAWING (5/18) from the front of the Room 206 door.

    Continue down the hall to the elevator. Press the call button on the left wall to trigger a cutscene.
Tips
  • If you look off the side without railing in the Operation Theatre Balcony, you can see Nurses below.
  • You can lure Nurses around the room and make them stop if Alex stands still with the light off.
  • Be careful moving around in the Operation Theatre; bumping into objects will alert Nurses.
  • During the transition to the next area after the elevator cutscene, Alex's health will be fully recovered. This is a good opportunity to take advantage of it, especially on hard mode.
