How to Reach the Karnok's Wall Ancient Obelisk:

First things first, you'll want to fast travel to The Ribs. From there, just follow the main path. Keep going until you reach a place called Fort Calcium. As soon as you get there, look over the edge. You should spot a bridge below you. Drop down onto that bridge, and it'll lead you straight to the Ancient Obelisk.

How to Complete the Karnok's Wall Ancient Obelisk:

Once you interact with the Obelisk, get ready for some snake-like enemies to show up. After you deal with them, the mini-boss for this area, Shara, Dust Begotten, will spawn.
