    After grabbing anything new from containers on the ground floor and upper floor, make your way to Dr. Swansea's office.

    During the subsequent conversation, answer however you please since your answers don't make a difference.

    Swansea will direct you speak to the hospital's benefactor, Lady Ashbury, who can be found near the tents.

    Before leaving Dr. Swansea's office, examine a side table to find Rare Species of Vampires (4/30).

    Look on the cupboard just above your current position for a Warning Letter which unlocks a hint about Thoreau Strickland and a Note to Dr. Swansea which unlocks a hint about Waverley Ackroyd.

    Cross over to the other side of the office and search the bench for a Letter of Rakesh Chadana which unlocks another hint about Rakesh.

    Back downstairs, you can explore new conversation topics with Strickland ("Because of your argument with Ackroyd?") and Ackroyd ("About Tippets' opinion on your promotion?").

    Back outside go around looting the newly stocked containers.

    If you make it round to where Rakesh Chadana is operating the morgue, you'll have a new conversation option, "Your life in London?" / "You were appointed as a medic?".

    When you're ready to proceed, follow the quest marker and approach Lady Ashbury for a scene.

    Answer her however you please. Select "I'm here to help you" to progress.

    Lady Ashbury will direct you to speak to Harriet Jones.

    Head down to the river to where a locked gate has previously barred your way. You will receive a tutorial on eavesdropping on Dorothy Crane. Go to the marked spot for a scene.

    After the scene, you will find yourself on the other side of the gate.

    Before heading into the sewer, you may want to return to your office and rest since you should have a lot of XP to spend; for reference, the source had 2975XP. Ideally, you want to get to around level 9 or so. The source took a point in Blood Barrier, a point in Body Condition and a point in Physical Prowess, holding 1175XP back.

    Head down into the sewers. The gate immediately ahead of you is locked from the other side so turn left and go through the gate there.

    Head down the stairs and loot a couple of crates for medical ingredients.

    Go through to the next room where Jonathan will comment about the carnage. There's a container here that you can loot and you can unlock the gate that was previously barring your way.

    Start following the blood trail. Kill the Skal on the upper level and then search near to where he was standing for a Common Barbed Cudgel (5/20) and 5/-.

    After killing the enemies on the lower level, loot a crate containing Bullet (x12).

    Go down the tunnel ahead.

    When you have a choice, go into the tunnel to your left. Kill the Skal in here and go up a couple of steps and loot a crate for Light Regeneration Serum (x2).

    Search a body on the ground for a piece of lore, The Ban of the Dragon (5/30).

    There's a hamper filled with medical ingredients and if you need to fill your blood meter, there are several rats scurrying around.

    Press forward and drop down for a fairly easy boss fight against the Sewer Beast.

    Search the body on the floor for Nurse Crane's Voucher to update your quest.

    On your way out of the boss chamber, grab some medical ingredients from another crate.

    Backtrack to the previous corridor and head up the stairs to your right.

Claustrophobia

  • This is a new type of mission. There is a citizen in danger. Head down to find two Skals menacing a man behind a locked door.
  • When the Skals are defeated, open the door and make yourself known to Oswald Thatcher.
  • You will earn 50XP for rescuing him and he will appear in the Pembroke Hospital after the next time you rest.
  • There's a crate in Oswald's refuge that you can loot and a couple more in the area where you found him.

Tool of the Trade

  • Head east along the dock. A level 11 Skal will leap out at you. After defeating it, loot the crate under one of the archways for some crafting materials. Go to the next arch along and search the crate for a Large Knife. You will also find 5/- on a shelf.
Tips
  • Containers in and around the hospital should have refilled. If you're not finding new containers and contents, try restarting the game.
  • Your answers during conversations with Dr. Swansea and Lady Ashbury don't make a difference.
  • The Barbed Cudgel makes the Sewer Beast fight straightforward. Three or four good whacks will leave the beast stunned. Do not use your Bite attack when it is stunned, however, because its stun bar will grow. Instead, simply recover your Stamina and wallop it again when it gets up. Keep an eye on your HP, since its attacks are quite damaging and the damage it does is aggravated damage, but if you are able to keep it stun locked, you should have no trouble defeating it.
  • The gate immediately ahead of you in the sewers is locked from the other side.
  • The tunnel to your left in the sewers leads to a grate that you can't get through.
