Phases
- When the Wall of Flesh spawns, you will be inflicted with the Horror debuff, preventing teleportation out of the Underworld. Attempting to do so results in instant death.
- While in the Underworld, leaving the biome or moving too far from the Wall of Flesh inflicts the The Tongue debuff. A tendril will attach to you, pulling you towards the mouth and dealing 50 damage per second.

- Run away from the Wall of Flesh on a long runway and attack its eye, which has the lowest defense. If the eye is obstructed, target the mouth parts.
- Destroy the Hungries to allow damage to the eye and mouth. They drop hearts that can aid in survival.
- The eye fires lasers, and the mouth spawns Leech enemies that also drop hearts.
- In Expert Mode, Hungries and Leeches drop hearts less frequently, making regeneration and potions more crucial. Hungries respawn, so piercing weapons are beneficial.
