Alright, so after you've dealt with Father Gascoigne, there's a little side-quest involving Viola's Daughter that you can pick up. Head to the top of the building where you found Gascoigne and grab the Red Jeweled Brooch. Bring it back to the little girl.
Now, you've got a choice to make: give her the brooch or keep it. This decision actually branches the questline and affects what rewards you get later.
If you return the brooch: The girl will vanish. The next time you reload the area, head into the sewers. You'll need to fight the Maneater Boar there to get the Red Messenger Ribbon.
If you withhold the brooch: You get to decide where to send her. You can send her to Oedon Chapel or Iosefka's Clinic. Each choice has a different outcome:
- Send her to Oedon Chapel: She won't make it there alive, unfortunately. You'll still get the Red Messenger Ribbon from the Maneater Boar, just like if you'd returned the brooch initially.
- Send her to Iosefka's Clinic: You'll get Lead Elixir x2 from Iosefka right away. However, she'll eventually turn into a beast due to Iosefka's experiments, and her questline won't continue further.
After defeating Rom, the Vacuous Spider
This part of the quest only matters if you managed to snag the Red Messenger Ribbon from either the boar or by sending the daughter to Oedon Chapel. Go back to that window in Central Yharnam where you first interacted with her. You'll find her older sister there. Hand over the Red Messenger Ribbon.
After that, reload the area. Head down the ladder from their house, and you'll find a White Messenger Ribbon on her corpse. It's a bit grim, but that's how you wrap up this particular quest.
