Walkthrough

At the start, kill the two Zombiemen in the cages if playing on skill levels 3 or 4. Do not blow up the barrel nearby.

Drop to the floor below, pick up the Shotgun and Box of Bullets, then blow up the barrel in the corner.

Proceed through the opening into the hallway and kill the Shotgun Guys.

Follow the hallway until a door opens behind you; kill the Demon that emerges on harder skill levels.

Open and go through the door in the hallway.

In the new room, kill the Zombiemen and destroy the barrel in the corner.

Take the left staircase. At the top, run to the small platform to your left, pick up the Box of Shotgun Shells, check the panel to reveal and pick up the Green Armor, then drop back down.

Take the right staircase. When you reach the conveyor belt, kill the Zombiemen that appear behind you when a window opens.

At the other end of the conveyor belt, run onto the platform to your right, hit the switch to open a door, then drop down.

Go back up the right staircase again. At the other end of the conveyor belt, go through the opening to your left.

Pick up the Chainsaw and go straight, returning to the hallway near the start.

Hit the switch across from you to lower a lift, get on it, and ride it to the top.

Destroy the barrel in the corner behind you. Kill any Zombiemen here on harder skill levels and kill the Demon across from you.

From the lift, take the first opening to your left. Kill any Zombiemen, turn right, and search the walls on your right until you find a hidden door.

Destroy the barrel inside and pick up the MegaArmor. Return to where the lift was.

Take the second opening to your left. If playing on skill levels 1 or 2, you will find the Blue Key.

If on skill levels 3 or 4, hit the switch, turn around, run straight, and take a right. If quick enough, you will see a small opening to your right with the Blue Key. If the door closes, go back and hit the switch again.

If you visit this place on skill levels 1 or 2, you will find a Berserk.

Once you have the Blue Key, return to where the lift was and drop back to the hallway below.

In this hallway, go through the opening to your left to find the Blue Door. Open it and go through.

Kill the Zombiemen inside and blow up the barrel across from you. Kill the Zombiemen in the cages to your left.

Go up the staircase in the corner of the room.

Go left, kill any Shotgun Guys, and hit the switch across from you to open a door revealing a room with Demons.

Kill all the Demons, then enter the room, pick up the Medikit, destroy the barrel, and hit the switch.

Leave this room and go back to the staircase. Go through the new opening to your left.

In this hallway, go left to an opening. Go through it, and the floor will lower.

Destroy the barrel and pick up the health bonuses. If needed, lower the lift, ride it to the top.

Kill any Shotgun Guys and open the door at the other end of the hallway. Kill all the Demons inside.

Go left and open the next door to find the exit room. Kill the Imp but do not hit the exit switch.

Blow up the barrel and leave the room. Go back to the previous hallway.

Go straight and follow the hallway to two openings. Do not go through the opening to the starting hallway.

Go back through the opening to your right. Get on the small step to your right.

Back up and run across the gap to the small platform with the switch. Hit the switch and drop down to the floor.

Run to the lift to the left of the cages and drop down it. Ride it to the top.

Pick up the Soulsphere and hit the switch. Ride the lowering lift to the bottom.

Blow up the barrel and step through the teleporter.

You will be back at the level's starting point. NOW blow up the barrel.

Turn around, run to the door, open it, go through it, and step through the exit teleporter to your left. If you were too late, the lift will rise and you will be unable to lower it again. If the lift doesn't lower after blowing up the barrel, you missed one or more barrels elsewhere.

To open the secret exit, you need to blow up all barrels EXCEPT the one at the starting point. Once all other barrels are destroyed, return to the starting point and blow up the barrel.

Tips

The Zombiemen in the starting cages are only present on skill levels 3 and 4.
Shotgun Guys can deplete your health quickly.
On harder skill levels, a Demon will emerge from a door behind you in the first hallway.
A hidden door revealing a MegaArmor is found by searching the walls on your right in the first left opening from the lift.
The Blue Key's location depends on the skill level: found directly on skill levels 1 or 2, or requires a timed sequence on skill levels 3 or 4.
A Berserk pack is available on skill levels 1 or 2 in the Blue Key area.
Be careful not to hit the exit switch in the final room before blowing up the barrel; this leads to the normal exit.
Missing barrels elsewhere in the level will prevent the lift from lowering at the start after blowing up the final barrel.
