From the start, find the first bridge with armor. Sidestep off while facing the armor to find an underwater hallway beneath it. Follow this hallway to a larger room, turn left, and enter the room with arrows pointing to its doorway. Continue to the end of this room and take the first right. You will go up stairs, out of the water, and into a secret area.

After obtaining the gold key, three doors will open, possibly with enemies appearing. Enter the rightmost room, and shoot the back wall to reveal an opening. Inside, collect the +100 health box. Jumping into the water here will reveal a yellow armor and a barricade.

At the level's final intersection, one path leads to the regular exit, and another to a room with multiple boxes. Go to the room with boxes and shoot the red switch on the ceiling. A door will open above the middle box. Enter, grab the +100 health box, and hit the other switch. This opens the barricade mentioned in secret #2. Return to the barricade, go through it, and up the ramp to find a teleporter leading to the secret level, The Underearth.

In the room with the arched bridge leading to the gold door, a zombie sniper is located on a distant platform. Use a well-aimed grenade to hit him. Upon his death, a wall to the immediate left of the gold door will open, granting access to the zombie's platform and his items.

Secrets:

The Well of Wishes: After traversing the spike hallway and crossing the bridge, turn around and look right. A dimly lit ledge with a Quad Damage pickup is visible. Jump to this ledge (aiming slightly right from the spike area exit is easier). Follow the narrow ledge around until you find a small area to jump into. This leads to a water pit, and upon entering, you will find yourself in the Well of Wishes, encountering DopeFish.
