The quickest way to get there is to head to Kyovashad, take the eastern bridge out, and just keep going east through those narrow passes. You're aiming for the big valley area. There's a hill path on the south side that leads right into the stronghold.

Finally, we have the Nostrava stronghold. This one's Level 23 and sits in the southwestern part of Fractured Peaks. If you've got the Nevesk waypoint unlocked, fast travel there and then head northwest along the road. Keep an eye out on the east side of the road; you'll find an abandoned settlement. Head inside that, and you'll discover the stronghold itself.
