The Holy Chalice spawning repeatedly in Kyovashad in Diablo 4 is a known bug. Fortunately, this issue does not affect gameplay or your overall progression, so you can safely ignore these extra Holy Chalices.

Normally, the Holy Chalice is a crucial item within the Malady of the Soul questline in Kyovashad. This questline includes the side quests titled 'Malady of the Soul,' 'Depths of Despair,' and 'Faith in Blood.' In these quests, the Holy Chalice is used at Sister Octavia's request for exorcism rituals, often culminating in battles against hellspawn. Typically, the Holy Chalice is removed from your inventory after its use in the ritual. However, due to a bug, it continues to spawn in Kyovashad specifically during World Tier 4 (Torment difficulty).

It is currently unknown if this bug is triggered only by starting the questline, while it's in progress, or if it persists after completion. Regardless, picking up these extra Holy Chalices does not impede the progression of the 'Malady of the Soul' questline, and any redundant copies will be automatically removed once you complete these associated side quests.
