Wrathful Malignant Hearts are the rarest type of Malignant Hearts in Diablo 4, and class-specific ones like The Great Feast are only obtainable in World Tier 4. These powerful items offer significant bonuses but often come with substantial drawbacks, requiring careful build consideration.

The Great Feast: This Wrathful Heart grants minions a +50-75% damage increase but drains 1.0-2.0 Essence per second per minion. If you have no minions, you gain the damage bonus at the cost of losing 5.0 Essence per second. The damage increase is additive. This Heart requires specific builds and careful Essence management.

How to Farm Wrathful Malignant Hearts

Wrathful Hearts cannot be directly crafted. Instead, you must rely on RNG through various methods:

Uncertain Hearts and Uncertain Invokers

At Cormond's Workbench, you can spend ichor to craft Uncertain Hearts (which may yield a Wrathful Heart) or Uncertain Invokers (which may yield a Wrathful Invoker). This method is entirely dependent on luck and requires running many Malignant Tunnels to gather ichor.

Wrathful Elites and Outgrowths

Malignant Elites, including Wrathful variants, can randomly spawn and drop the appropriate type of Malignant Heart. Additionally, enemies can drop Wrathful Invokers, which can be used on Wrathful Outgrowths found at the end of Malignant Tunnels for a chance to obtain a Wrathful Heart.

Malignant Tunnels

Running Malignant Tunnels repeatedly is the most consistent way to farm the necessary resources (ichor for crafting Uncertain items) and encounter Wrathful Elites or Invokers. It's recommended to tackle these in World Tier 3 or higher.

Urn of Malignance

Upgrading the Urn of Malignance using Smoldering Ashes can increase the drop rate of rare Malignant Hearts, potentially improving your chances.

Echo of Varshan

Defeating the Echo of Varshan can drop a random Malignant Heart. However, the chance of getting a specific Wrathful Heart is low (around 1-in-32), and this method requires specific Invokers and is more challenging than farming Malignant Tunnels.
