The first way you'll probably stumble upon these is by using your Blood Press. You can take 3 Exquisite Hearts and refine them into 1 Primal Blood Essence. Now, if your Blood Press isn't sitting on matching Alchemy Lab Flooring, it'll cost you an extra Exquisite Heart, so 4 total. Exquisite Hearts are pretty rare drops, so start hoarding any hearts you find in your castle stashes – you never know when they'll be useful!
