Second Floor: Finding Leonard

At the end of the patient wing, find a mysterious door labeled "M" with no number.

Enter this secret path, which has no map (minimap (the small circular map in the bottom-left corner of your screen)).

Follow the path as gates open and close. If a gate doesn't open, another path will present itself where a gate closes, which then opens a previous gate.

Make your way through this maze path and through the door at the end.

Proceed through the second corridor in the same manner.

In the third corridor, you will reach a path where six gate doors close behind you, and one blocks the way forward.

Examine the red symbol on the wall, known as the "Mark of Samael" (from the first Silent Hill). This is not a save point (checkpoint (an auto-save point you restart from on death)).

Examine the Mark of Samael for a brief flashback of Lisa from the first Silent Hill.

Proceed through the door that newly appears down the hallway.
