- 1In Hardcore mode, you must manage HP, RADs, water (H2O), food (FOD), and sleep (SLP). Notifications appear as icons above your HP bar, and detailed levels can be checked via the Pip-Boy.
- 2Boost your Survival skill to 80 as soon as possible. This enhances food and water effects and allows for superior campfire meals.
- 3Water is the most frequently depleted need. The Vault 13 Canteen (from the Ultimate Edition) helps but doesn't eliminate the need. Free clean water sources include casinos in New Vegas (like the Atomic Wrangler), troughs at Goodsprings Source, and the Colorado River.

Bighorner Steak heals about 50 HP and is more convenient than Stimpaks. The Monocyte Breeder Implant provides slow health regeneration. Fast-traveling also offers supplemental healing.
- 7At level 20, acquire the perk ‘Them’s Good Eatin’’. This causes living enemies to drop Blood Sausage or Thin Red Paste 50% of the time. These items heal more HP than a Stimpak and are abundant. Thin Red Paste weighs only 1/10th of a pound.
