Render Unto Caesar

Eliminate Mr. House: This objective should have been completed earlier. Refer to the section “The House Always Wins (Or Not)” under the heading “The House Has Gone Bust”.

Secure Boomer Support: Return to Caesar after dealing with Mr. House. He will task you with gaining the Boomers' support. This is achieved by completing the quest Volare!. Alternatively, you can kill Mother Pearl and Loyal.

Secure White Glove Society Support: After dealing with the Boomers, return to Caesar. He will ask you to gain the White Glove Society's support. This involves completing the quest Beyond the Beef. If you prevented them from reverting to cannibalism, inform Caesar. Otherwise, ensure Mortimer's cannibal scheme is confirmed, then secure Marjorie's agreement to an alliance, and report back to Caesar.

Destroy the Brotherhood of Steel: Caesar's next task is to eliminate the Mojave chapter of the Brotherhood of Steel.

Option A: Self-Destruct: Steal the three Keycards from the Elders (Head Scribe Taggart, Head Paladin Hardin, Elder McNamara). Use these to generate a self-destruct password at the Self-Destruct Authorization Terminal in the Hidden Valley Bunker L2. Activate a nearby Self Destruct Terminal with the password to initiate the bunker's self-destruction. You will gain the “Vilified” reputation and must escape before the explosion. Wearing Brotherhood of Steel attire (e.g., Head Scribe Taggart's) can ease your escape.

Option B: Direct Assault: Murder all members of the Brotherhood of Steel within the bunker.

Deal with Caesar's Illness: After completing the previous tasks, return to Caesar. He will admit he is sick. You can diagnose him with a [Medicine 40] or [Speech 40] check. He has a tumor.

Option A: Perform Surgery: Pass a [Medicine 75] or [Speech 65] check to offer surgical assistance. If you pass the [Medicine 75] check, you can perform the operation.

Option B: Find Diagnostic Module: If you cannot perform surgery, travel to Vault 34. Locate the Broken Mk III Auto-Doc in the hospital room and salvage the Diagnostic Scanning Module.

Return to Caesar: Obtain the necessary items (Surgical Tools and Doctor's Bag from Dr. Usanagi, or the Diagnostic Scanning Module from Vault 34). Return to Caesar's tent at The Fort. You will find that Caesar has fallen into a coma.
