Follow this guide to navigate the Amara Deep Zone, make the crucial choice for the True Demon Ending, and prepare for the final battle in Shin Megami Tensei III: Nocturne HD Remaster.

You'll find yourself back in the first "Mystery Area" of the game, located in the hospital. In the same area where you found the Maharagi Stone treasure chest, a Shinentai will approach you. This entity will ask to examine your soul to determine if you will defy fate and face the final battle against God, or allow history to repeat itself. If you allow the Shinentai to look into your soul, it won't provide a direct answer, instead stating that all life is fated to end and that humanity is manipulated by fate. It poses a hypothetical question: would you hold a grudge against God for creating such a fate if a loved one died in a tragic accident? The Shinentai suggests that if you are dissatisfied with the current order and wish to break free from fate's chains, you should seek out the "great one.".

In the room on the left branch of the corridor, you'll find a lift that descends to the deepest part of the Amara Deep Zone. Once you descend, there is no turning back. If you wish to obtain one of the original "Creation" endings in Nocturne, you must leave now, as descending will lock you into the path leading to the "new" ending and the new secret final boss. If you have already entered Kagutsuchi Tower, you are out of luck, as the lift will not activate, preventing you from accessing the new ending. Additionally, you can only descend once; the lift will not function if you return after your initial descent.

Upon descending the lift, you will arrive directly in the chamber with the old gentleman in the wheelchair and his nurse. The nurse expresses elation that you are willing to sacrifice the last remnants of your humanity to support their cause. She states that their long wait throughout the aeons has finally borne fruit, as you have collected all the Menorahs and defeated the strongest assassin sent by the Light. She proclaims you are the demon they have been awaiting. The observing demons watching through various peepholes erupt in cheers until the old man signals them to cease. The nurse asks if you can hear their cheers, signifying that you have brought them a ray of hope and will join the "dark angel" in his fight. She emphasizes that there is no turning back now, as the conflict between those cast out from the light and those who control it is about to commence. Your final task is to defeat the Guardians who seek to establish their Kotowari, and then Kagutsuchi itself, to shatter the cycle. Once you accomplish this, they will join you in the final battle, and her master will bestow a portion of his power upon you. Subsequently, the old gentleman in the wheelchair unlocks the power of your first Magatama (Marogare). The nurse declares, "You were given the body of a demon by the Great One, and now, my master will give you the soul of one, too. Go, our new demon, our new hope!" as the old man completes your transformation, turning you into a 100% demon. You will then awaken again, reborn (as alluded to on the back of the game's box: "the demon is born twice") in Shinjuku hospital.

The Amara Deep Zone is now complete, but you can return later. (And you should, to retrieve the items behind the True Demon door and open the Metatron/Beelzebub doors, assuming you haven't already.) At this point, your only remaining objective is to complete the game.
