Shrine of Shackles

From Olivia's Promontory, board your ship and sail west. After a (cutscene (a non-interactive cinematic the game plays automatically)) (a non-interactive scene that advances the story), continue sailing west to find the Shrine of Shackles on a small island.

You can speak with the Flame at the beginning of the jail area.

Follow the path south to the first intersection.

Go west and enter the cell. Search the pot inside to find Mini Medal #68.

Return to the hallway and continue south to the bottom of the floor.

Go west to the southwesternmost cell. You will witness a (cutscene) of Simao and Ortega sparring.

Search the pot in this room for 4 gold.

Go east to the southeasternmost cell and enter it for another (cutscene).

You will be rewarded with the Mountaincleaver weapon.

Search the pot in this room for the Weightlifter's Belt (Tough Cookie personality).
