Walkthrough

Go southwest to find a ruined house on the right side of the street. This is the area mentioned in "Form Alpha-One".

Duck under the boards to enter the house.

Stare at the wall by the crate-filled doorway with Mind's Eye (a skill that reveals hidden things) to dispel the illusionary wall. Clearing the infestation will make it easier to use Mind's Eye. You will gain +141 XP [SKILLS].

Head upstairs and turn around.

Stare at the wall to the left of the stairs with Mind's Eye to dispel another illusionary wall.

Drop down the crates ahead and climb the crates around to the right to get back to the upper floor.

Turn around to encounter a table and chair.

Interact with the cylinder on the chair. This will grant you Form Alpha-Two [CASEBOOK, Functional Brain Cylinders], E. Waite [LORE, Brain Cylinders] (sc06), and mark Key Evidence Collected. You will also gain +184 XP [SKILLS].

Go around the two holes in the floor to the front of the house and drop down to the roof of a car to leave the infested house.
